Double guillotine, I stick with Xikar
V cut - I don't do a full deep V cut, not a fan as they can easily crack your wrapper since it's a single blade pushing to make the cut, especially if you cut a full deep V. Two moderate cuts in an X gives a decent draw too.
A small sharp pocket knife, rotate around the cap carefully to just remove the cap.
Never ever ever teeth. That ****e is for old Westerns, good way to kill a good cigar.
Not a punch fan unless it's all that's available, tar always seems to be an issue with a punch cut for me and interferes with the draw as the cigar progresses. A dull punch is also a good way to damage your cigar.
Observe where the cap ends and stay at least 1/8" above it or you start to unravel.
Don't lick or suck on your cigar first, just makes the cut messy.
Perforators SUCK.
